Team Leader* – Supported Living

Avicenna Supported Living:
We are looking for an experienced Team Leader to join our Supported Living team, supporting adults with learning disabilities, autism and complex support needs.
This is a hands-on leadership role combining direct support with responsibility for guiding staff, maintaining high standards of care and supporting the day-to-day running of the service.


The Role
You will:
  • Lead and support staff during shifts.
  • Write, review and update care plans and risk assessments.
  • Support the implementation and review of Positive Behaviour Support (PBS) plans.
  • Complete staff supervisions and competency assessments.
  • Monitor care records and ensure documentation is accurate and up to date.
  • Support medication management, safeguarding and quality assurance.
  • Coach and guide staff to maintain good practice.
  • Work closely with management, families and other professionals.
  • Provide direct support when required and promote independence, choice and positive risk-taking.

About You
Essential:
  • Level 3 NVQ/Diploma in Health & Social Care or equivalent.
  • Good experience within adult social care, ideally learning disabilities and/or autism.
  • Previous senior, supervisory or leadership experience.
  • Working knowledge of Positive Behaviour Support (PBS).
  • Experience writing or reviewing care/support plans and risk assessments.
  • Good understanding of safeguarding and person-centred support.
  • Confident supporting, guiding and developing staff.
  • Good written, verbal and IT skills.
Desirable:
  • Experience completing staff supervisions and competency assessments.
  • Experience supporting people with complex needs or behaviours of concern.
  • Medication and quality assurance experience.
  • Full UK driving licence and access to a vehicle.

  • We want someone who is calm, organised, approachable and confident taking responsibility.
  • You'll need to be comfortable leading by example, challenging poor practice when necessary and supporting your team to provide safe, consistent and genuinely person-centred support.
  • This isn't an office-based management role. We want a Team Leader who is visible, hands-on and knows both the people we support and the staff team.
